A scheduled appointment date is required for standby status.
Standby vaccinations are prioritized by age.
Please arrive a few minutes prior to your scheduled standby appointment time.
When you arrive at the Cottage Health Vaccination Clinic, let the staff know you are there as a “Standby” appointment.
If you arrive and you do not receive a vaccine on your standby appointment date, we will schedule your vaccination for our next available clinic day.

Instructions:

Click on an available date and time below.
On the next page, enter “COVID Vaccine” as your Reason for Visit and click “CONTINUE”
If you have a Cottage MyChart account, “LOG IN,” otherwise click “CONTINUE” as a guest.
You will be asked to provide your patient information and click “NEXT.” Please use the PATIENT's information.
Once you see the page that says, “Your appointment is scheduled” the process is complete.

Additional information:
If you have received monoclonal antibodies (Bamlanivimab or Casirivimab/Imdevimab) for the treatment of COVID-19 in the past 90 days, before scheduling an appointment please contact your physician to discuss whether you should receive the COVID-19 vaccine.

Persons who have had a known COVID-19 exposure should not seek vaccination until their quarantine period has ended, to avoid potentially exposing healthcare personnel and other persons to COVID-19 during the vaccination visit.

Second dose instructions:
This Cottage vaccine clinic is only administering the Pfizer COVID-19 vaccine. If your first dose was the Moderna COVID-19 vaccine, please schedule your second dose with an organization administering Moderna vaccinations.

If you receive the Pfizer vaccine from Cottage Health, you will receive email reminders for scheduling your second dose. Please ensure you are scheduling your second dose at least 17 days after your first dose of COVID-19 Pfizer vaccine. The recommended interval between doses is approximately 3-6 weeks, no earlier than 17 days.
